     1.   Issuance of Simple Subdivision Approval

                A. ABOUT THE SERVICE

                  In accordance with Section 4 Presidential Decree No. 957 and its implementing Standards, Rules and Regulations, as amended by Executive Order No. 648 dated 07 February 1981 the approval for the development of a Simple Subdivision Project should conform to the Approved Comprehensive Land Use Plan of the Municipality.

     Application for Approval of a Simple Subdivision Project under PD 957 

  • Four (4) sets of Subdivision scheme plans showing the proposed subdivision layout, lot frontage dimension, aggregate lot areas.
  • Four (4) sets of Vicinity Map/Location Plan drawn to scale, showing the adjoining land uses and circulation  network as well as the existing landmarks, facilities and utilities at least 100 meters from project boundaries.
  • Three (3) certified copies land title (OTC/TCT) and tax declaration (Deed of Sale /Memorandum of Agreement if the title is not registered in the name of applicant). 

 

    2. P R E L I M I N A R Y   A P P R O V A L   and   L O C A T I O N A L   C L E A R A N C E   ( P A L C) 

 

      A. Application for Preliminary Approval and Locational Clearance of Projects under B.P. No. 220

 

  •  Five (5) sets of Subdivision scheme plans showing the proposed subdivision layout, lot frontage dimension, aggregate lot areas
  •  Five (5) sets of Vicinity Map/Location Plan drawn to scale, showing the adjoining land uses and circulation  network as well as the existing landmarks, facilities and utilities at least 100 meters from project boundaries.
  •  Five (5) certified copies land title (OTC/TCT) and tax declaration (Deed of Sale /Memorandum of Agreement if the title is not registered in the name of applicant). 

 

      B. Application for Preliminary Approval  and Locational Clearance of Projects Commercial & Industrial  Subdivision

 

  •  Two (2) sets of Site Development Plan (Schematic Plan) at a scale ranging from1:200 to 1:2,000 showing the proposed layout of streets, lots, parks and playgrounds and other features in relation to existing conditions in the area duly signed and sealed by any Licensed and Registered Architect, Environmental Planner, Civil Engineer or Geodetic Engineer.
  •  Two (2) sets of the following documents duly signed by a Licensed and Sealed by a Licensed Geodetic Engineer.

A. Vicinity Map indications the adjoining land uses, access as well as existing facilities and utilities at less within 500 meters from the  property boundary of the project, drawn to scale.

B. Survey Plan of the lot(s) as described in TCT(s).

C. Topographic Plan

  •    Two (2) copies of Certified True Copy of Title(s) and Current Tax Receipts.
  •    Right to use or deed of sale of right-of-way for access road and other utilities when applicable.
  •    Certification as to the Zoning Classification of the Project Area from the LGU.

 

    C. Application for Preliminary Approval & Locational Clearance for Memorial Park / Cemetery

 

  •    Two sets (2) of the following documents duly signed and sealed by a licensed architect/ civil 

                      a.Subdivision plan showing the proposed lay-out.

b.Vicinity map within a minimum of one hundred (100) meters from the periphery of the project showing the relationship of the proposed project existing residential structures, sources of water supply, national roads and community facilities.

  •    Two sets (2) of survey plans of lot(s) as described in the TCT(s) duly signed and sealed by a licensed geodetic engineer.
  •    Certified True Copy of Title(s) and Tax Declaration(s).
  •    Sangguniang Panglunsod/Bayan Resolution stating the necessary of the project in relation to the needs of the locality.
  •    Clearance from the Department of Health.
  •    Certification from NWRC that the proposed site is on the ground where the water table is not higher than 4.25 meters below the ground       surface.

 

   D. Application for Development Permit under BP 220 & PD 957 Projects Commercial, Industrial, Memorial/ Cemetery,                    Farmlot 

  •  All documents required in the application for Preliminary Approval and Locational Clearance (in case combined PALC and DP application.
  • Subdivision Development Plan at scale, ranging from 1:200 to 1:2000 prepared, signed and sealed by any Licensed, and Registered Architect, Environmental Planner, Civil Engineer or Geodetic Engineer.
  • Civil and Sanitary Works Design

                Engineering plans, construction drawing based on applicable engineering code and design criteria to include the following:

                     a.  At least 2 copies of road (geometric and structural) design/plan duly signed and sealed by Licensed Civil Engineer.

      1. Profile derived from existing topographic map showing the vertical control, designed grade, curve elements and all information needed for construction.
      2. Typical roadway sections showing relative dimensions of pavement sub-base and base preparation, curbs and gutters, sidewalks, shoulders benching and others.
      3. Details of miscellaneous structures such as curb and gutter (barrier, mountable and drop), slope protection wall, rip rapping and retaining wall.

                     b. At least 2 copies of storm drainage and sanitary sewer system duly signed and sealed by Licensed Sanitary Engineer or Civil Engineer.

      1. Profile showing the hydraulic gradients and properties of sanitary and storm drainage lines including structures in relation with the road grade line.
      2. Details of sanitary and storm drainage lines and miscellaneous structures such as various types of manholes, catch basins, inlets (curb, gutter and drop), culverts and channel linings.
      3. At least 2 copies of site grading plan with the finished contour lines superimposed on the existing ground the limits of earthwork embankment slopes, cut slopes, surface drainage, drainage outfalls and others, duly signed and sealed by a Licensed Civil Engineer.
  • At least 2 copies of water system layout and details duly signed and sealed by a Licensed Sanitary Engineer or Civil Engineer. Should a pump motor have a horsepower (HP) rating of 50HP or more, its pump rating and specifications shall be signed and sealed by a Professional Mechanical Engineer.
  • Certified true copy of Tax Declaration covering the property (ies) subject of the application for the year immediately preceding.
  • Certified true copy of Environmental Compliance Certificate (ECC)or Certificate of Non-Coverage (CNC) duly issued by the DENR, whichever is applicable.
  • Zoning Certificate from HLURB Regional Office.
  • Certified true copy of DAR conversion order.
  • At least 2 copies of project description for projects having an area of 1 hectare and above to include the following:
    1. Project profile indicating the cost of raw land and its development  (total project cost), amortization schedule, sources of financing cash flow, architectural plan, if any, and work program;
    2. Audited financial statement for the last 3 preceding years;
    3. Income tax return for the last preceding years;
    4. Certificate of Registration from Securities and Exchange Commission (SEC);
    5. Articles of incorporate or partnership;
    6. Corporate by-laws and all implementing amendments; and
    7. For new corporations (3 years and below) statements of capitalization and sources of income and cash flow to support work program.
  • Application for permit to drill from the National Water Resources Board (NWRB).
  • Traffic impact assessment (TIA) for subdivision projects 30 hectares and above.
  • List of names of duly licensed professional who signed the plans and other similar documents in connection with applicable filed indicating the following information:
      1. Surname;
      2. First Name;
      3. Middle Name;Maiden Name, in case of married women professionals;
      4. Professional License number, Date of Issue and Expiration of its validity;
      5. Professional Tax Receipt and Date of Issue;
      6. Taxpayer’s Identification Number (TIN).
