Pls. bring the following documents during the renewal of business permit:

1. Filled up – application form
2. Barangay business clearance
3. Monthly gross income statement CY-2012
4. Post inspection reports from sanitation, CPDC, OBO & TREAS.

Clearances:

Health Certificate & Sanitary PermitRealty Tax Clearance (if no property, secure a cert. from Assessor’s Off. & Contract of Lease)Annual Building Inspection ClearanceZoning Clearance / CertificationFire ClearanceBIR (TIN, Payment of Annual Reg., Certificate of Reg.), SSS, PHILHEALTH, PAG-IBIG

Note: Deadline for the submission of clearances is on April 21st of the Year. Submit previous year audited Financial Statement or ITR not later than May 20th of the Year.

Deadline of Quarterly Payments:

1st Qtr. – Jan. 20 3rd Qtr. – July 20

2nd Qtr. – April 20 4th Qtr. - Oct. 20
