1. Issuance of Service Record

This service is for preparing Service Records for employees, both current and past (including separated employees), upon request by the respective employee or their authorized representative.

  • Office/Division: Human Resource Division (HRD)
  • Classification: Simple
  • Type of Transaction: G2C (Government to Citizen)
  • Who may avail: Existing and former employees

Checklist of Requirements:

  • Authorization Letter (if the requestor is not the employee)
  • ID of the employee and authorized person (SSS, GSIS, COMELEC, Passport, Postal, or any government-issued ID)

2. Issuance of Certificate of Employment

This service provides a certification stating that an individual has been employed by the agency at a particular period, along with their pay/rate and duties (if applicable).

  • Office/Division: Human Resource Division (HRD)
  • Classification: Simple
  • Type of Transaction: G2C
  • Who may avail: Existing and former employees

  • Authorization Letter (if the requestor is not the employee)

3. Issuance of Certificate of No Pending Administrative Case

This service provides a certification stating that there are no pending administrative cases against the employee in the HRD records.

  • Who may avail: Existing and former employees

4. Filing of Grievance

This service is for facilitating and processing the filing of grievances against the agency or any official within the agency regarding mismanagement or unfair practices in the office.

  • Who may avail: All employees
